This guide walks the whole route end-to-end in current 2026 terminology — every exam, who can sit it, format, fees, and how long the realistic timeline is for both UK trainees and international medical graduates (IMGs).

Quick name check: why does every guide call these exams something different?

Because RCEM has renamed the exams twice in a decade. You need to recognise the legacy names because forum threads, question banks and old RCEM PDFs still use them.

• MRCEM Primary = was called FRCEM Primary (Aug 2016 – Jul 2021) and MRCEM Part A before that.
• MRCEM SBA = replaced FRCEM Intermediate SAQ in Aug 2021. Was once called MRCEM Part B.
• MRCEM OSCE = unchanged in name. Was once called MRCEM Part C.
• FRCEM SBA = replaced the FRCEM Final SAQ in Aug 2021.
• FRCEM OSCE = the final practical, currently UK-only.

If a resource or examiner says “Part A” they mean MRCEM Primary. “Intermediate” means MRCEM SBA. “Final SAQ” means FRCEM SBA. Treat anything older than August 2021 with caution.

What is the full RCEM exam sequence from Primary to Final?

Five examinations, in this fixed order for MRCEM, then a flexible pair for FRCEM:

1. MRCEM Primary — basic sciences theory (single best answer paper).
2. MRCEM SBA — clinical theory across the full EM curriculum.
3. MRCEM OSCE — 16-station clinical and communication OSCE. Passing this awards MRCEM.
4. FRCEM SBA — theory paper covering the full EM 2021 curriculum plus research, management and quality improvement.
5. FRCEM OSCE — 16-station OSCE on complex and challenging EM scenarios. Passing both FRCEM components (in either order) awards FRCEM.

You cannot reorder the MRCEM exams — Primary must come before SBA, and OSCE must come last. The FRCEM SBA and FRCEM OSCE are the only two you can sit in either order.

How long does the whole MRCEM and FRCEM pathway take?

For a UK trainee on the EM run-through, the route looks roughly like this:

• Years 1–3 (ACCS-EM): Sit MRCEM Primary in the first year, then MRCEM SBA across years 2–3, then MRCEM OSCE before progressing to ST4.
• Years 4–6 (Higher Specialty Training, ST4–ST6): 12 months of HST-level EM unlocks FRCEM SBA and OSCE. Most trainees aim to finish both in ST5 or early ST6.
• CCT: Awarded at the end of ST6 once both FRCEM components and all curriculum SLOs are signed off.

Total time from F2 → CCT is typically 6–7 years, assuming no out-of-programme time and a clean run of exams.

For an IMG outside the UK, MRCEM is most commonly used as a GMC registration route. Once you have your PMQ and registration, the realistic window to clear all three MRCEM exams is 12–24 months if you can sit each diet at the first available opportunity. Add another 1–2 years before FRCEM, because FRCEM SBA/OSCE need 12 months of HST-equivalent EM experience that most IMGs build after taking up an NHS post.

What is MRCEM Primary and who can sit it?

MRCEM Primary is the entry-level theory exam — 180 single best answer questions in 3 hours, mapped to the RCEM Basic Sciences Curriculum (anatomy, physiology, pharmacology, microbiology, pathology, evidence-based medicine).

Eligibility (2026):

• Hold a GMC-accepted primary medical qualification (PMQ).
• Hold current medical registration — provisional or full. This is the only RCEM exam you can sit on provisional registration, which is why F1/PGY1 doctors and IMGs at the start of the route can attempt it.

Delivered by Surpass at test centres worldwide, twice a year. 2026 sittings: 22 April and 27 October.

What is MRCEM SBA and how is it different from FRCEM SBA?

The MRCEM SBA tests the same EM 2021 curriculum that the FRCEM SBA does, but pitched at the Year 1–3 SLO level — i.e. what a senior EM SHO/registrar should know. 180 SBAs across two 2-hour papers with a 1-hour break.

The FRCEM SBA covers the full curriculum including research, management and QI, and is pitched at the level of an EM consultant ready to be signed off for independent practice.

Eligibility for MRCEM SBA:

• GMC-accepted PMQ with full registration and licence to practise.
• At least 2 years of post-PMQ medical experience (internship/house jobs count).
• Passed MRCEM Primary.

What is MRCEM OSCE and where can I sit it?

The MRCEM OSCE is the only practical examination on the MRCEM route. Sixteen 8-minute clinical stations with a minute of reading time each, plus two rest stations — total exam time around 2 hours 42 minutes. Stations test history-taking, examination, procedural skills, resuscitation and communication.

Since September 2022 it has used domain-based marking rather than checklist marking, the same approach as the FRCEM OSCE. The pass mark is set via borderline regression plus 1 SEM.

Eligibility:

• Full GMC registration with licence to practise.
• Passed both MRCEM Primary and MRCEM SBA.
• At least 6 months FTE Emergency Medicine experience above Foundation level (post full registration).

Locations: London, Chennai, Hyderabad, Kuala Lumpur. India and Malaysia sittings are markedly more expensive than London.

Passing this exam awards MRCEM and full Membership of the College.

What is FRCEM and when can I start the FRCEM exams?

FRCEM (Fellowship) is the senior exit qualification for EM (see FRCEM vs MRCEM examination system explained). It is two exams — FRCEM SBA and FRCEM OSCE — that you can sit in either order. Most UK trainees sit SBA first because the OSCE has limited London-only capacity, but there is no rule.

Eligibility for both FRCEM SBA and FRCEM OSCE:

• GMC-accepted PMQ + full GMC registration with licence.
• Completed MRCEM (or holders of the legacy FRCEM Intermediate Certificate within the last 7 years).
• If UK trainee: 12 months FTE EM at Higher Specialty Training (HST) level or equivalent.
• If non-trainee: 4 years FTE EM training, with at least 1 year at HST level.

FRCEM OSCE is currently UK-only, delivered in London twice a year. The FRCEM SBA runs worldwide at Surpass centres in May and October.

To pass the FRCEM OSCE you must achieve the overall pass mark and pass at least one resuscitation station — fail the resus station and you fail the exam regardless of overall score.

What is the 7-year currency rule and why does it matter?

RCEM updated its exam currency rules in 2025. The clock now runs in three stages:

• From passing MRCEM Primary you have 7 years to complete MRCEM SBA and MRCEM OSCE.
• From completing MRCEM you have 7 years to pass your first FRCEM exam.
• From passing your first FRCEM exam you have 7 years to complete the second.

If you exceed any of these windows, expired components must be retaken. Candidates who passed MRCEM Primary more than 7 years before the rule change have been auto-extended by 24 months. Extensions for genuine reasons — parental leave, long-term sick leave — are considered case by case by the Chief Examiner; email exams@rcem.ac.uk before your window expires.

How much do the RCEM exams cost in 2026?

Fees vary by exam, by membership status, and (for OSCEs) by country. RCEM members pay materially less, which is why most candidates join the College before applying.

• MRCEM Primary, MRCEM SBA, FRCEM SBA: £429 (UK member) / £485 (international member) / £525 (UK non-member) / £609 (international non-member).
• MRCEM OSCE UK and FRCEM OSCE UK: £586 member / £695 non-member.
• MRCEM OSCE India: £987 member / £1,186 non-member.
• MRCEM OSCE Malaysia: £1,121 member / £1,345 non-member.

Membership must be paid at least 24 hours before the application window opens to qualify for the discount. Local digital service tax (VAT/GST) may be added based on your country of residence — not the country where the exam is taken.

How and when do I apply for an RCEM exam?

Applications open at 10:00 UK time on the published date and close at 16:00 UK time seven days later. Late applications are not accepted, and OSCE diets in particular sell out fast. Specific 2026 application windows for every diet are listed on the RCEM exam calendar.

Reasonable adjustments (extra time, rest breaks, separate room, access to medication, screen overlays, etc.) must be requested within the application form with supporting evidence from a relevant professional. Requests made after the application window closes are rarely approved.

Which RCEM exam should an IMG sit first?

Start with MRCEM Primary. It is the only exam you can sit on provisional registration and from any country. Most IMGs use it as the first proof of credibility on a CV when applying for NHS posts, even before completing MRCEM.

The common IMG pain points to plan for:

• You cannot book MRCEM SBA until you have full GMC registration and a licence to practise, plus 2 years of post-PMQ experience. Most IMGs achieve this after their internship plus an additional year of clinical work.
• MRCEM OSCE requires 6 months FTE EM above Foundation level post-registration — for most IMGs this means doing an EM SHO/registrar post in the NHS or an equivalent role overseas.
• FRCEM is hard to access without 12 months of HST-equivalent EM (1 year at registrar level), so the realistic IMG route is MRCEM → NHS registrar post → FRCEM during or after CESR/Portfolio Pathway work.

Do I need MRCEM and FRCEM to work as an EM consultant in the UK?

To be entered on the GMC Specialist Register in Emergency Medicine via the standard route, you need a CCT, which requires FRCEM. The Portfolio Pathway (formerly CESR) does not strictly require FRCEM, but in practice almost every successful Portfolio applicant holds it because it covers most of the curriculum evidence in one go.

You can work as a registrar, specialty doctor or specialist in EM with MRCEM alone — and many IMGs spend several years in those roles before deciding whether to pursue FRCEM.

What happens after I pass FRCEM?

Passing both FRCEM components awards Fellowship of the Royal College of Emergency Medicine and the right to use the post-nominal FRCEM. Frequently asked questions

Can I sit MRCEM SBA before MRCEM Primary?

No. MRCEM Primary is a prerequisite for MRCEM SBA, and both are prerequisites for MRCEM OSCE. The MRCEM exams must be taken in order.

Can I sit FRCEM SBA before MRCEM OSCE?

No. You must hold MRCEM (i.e. all three MRCEM exams passed) before you can apply for either FRCEM component.

Can I sit the FRCEM OSCE before the FRCEM SBA?

Yes. The two FRCEM exams can be taken in either order. Some candidates prefer to clear the OSCE first because there are only two London sittings a year and capacity is tight.

How many attempts do I get at each RCEM exam?

RCEM does not cap attempts numerically, but the 7-year currency rule effectively limits how long you have between components. Expired components must be retaken.

Is negative marking used in any RCEM exam?

What English language standard does RCEM expect?
